There will be some stigma and it could stop her from getting some media jobs in the future especially if enough of the right people complain. The idol Dekami-chan even mentioned in an interview somewhat recently that one of several reasons for her own name change was that just being called "Paipai Dekami" was suggestive enough to cause a few cancellations. I can imagine that someone doing a quick image search in the future could also cause some problems for her while she's acting under the name Kominato Yotsuha. TV programs still have AV talent on for different segments or as personalities though, so she wouldn't really be locked out of the mainstream if she also makes a name for herself there. I just wouldn't expect any guarantees that she'd be part of a Fairies reunion unless maybe she's introduced only by her previous name some years down the road to try and maintain some separation between her different career paths.

@ the bold part, I would imagine that would be somewhat hard for a proper Fairies reunion in the future (unless the fans are accepting of her career switch). I would believe it will be hard to maintain some kind of separation? I mean at this time of age, as you said, it will be easy to identify her AV work or even the news of her going into AV (since she sort of make an announcement for it publicly).
I wonder how are the Fairies' fans reactions to this news.
